I have access to a private, quiet space for a video call

I have a reliable internet connection and a device with a camera

I understand the appointment is via video, not in person

Yes I have a referral (or my GP will send one)

History of psychiatric hospital admission

Occasional or minimal (e.g. yearly, social)
